As part of my Listening Kits, I've included new cup games in grades 3/4/5 that are to be played with the listening selections in the kits. They are really a great way to teach form. I may have a few more by the time these go to print.

In Grade 3
1 Entry Of The Gladiators, Julius Fucik
16 Favorite Rag, Scott Joplin
In Grade 4
1 Stars And Stripes, John Philip Sousa
3 Fig Leaf Rag, Scott Joplin
In Grade 5
14 Radetzky March, Johann Strauss Sr.

In addition to cup games, there are many other ways to have children actively listening, and I've categorized the listening pieces so it is easy to find one to teach rondo form, or ABA etc. Here is a partial list from Grade 5.
