Here's the mini composer report form that I'm using in the composer profile folders. I placed the pages of information on each composer in plastic sleeves in duotang folders. In the front pocket of each folder is the report form to be filled out by the student after reading the article. Following is how I have it laid out:

Read the article carefully, then answer each question as thoroughly as possible. Use complete sentences wherever possible. Place completed report back in the folder and place on teacher's desk when finished.

  1. What is the full name of the composer you've selected?
  2. What country is this composer from?
  3. Date of birth. Date of death.
  4. During which period in music history did this composer contribute?
  5. What instruments did s/he play?
  6. Name three of his/her major compositions.
  7. Describe two significant events that occurred during this composer's life.
  8. What was one of the most important musical contributions made by this composer. Explain why.
  9. Describe one interesting fact about this composer and/or his/her music.

Your report must be complete to receive participation credit in music for the above date. Should you not finish this mini-report during class time today, you may finish it today in the music room during lunch recess. Return finished/unfinished reports to the teacher's desk before leaving class.
