I had to develop this for a Master's program. Perhaps you can adapt it. I put it in a table format and used three levels of mastery: Exceeds, Meets, or Does Not Meet

Title: Music Literacy Unit
Working Together: (Collaboration and Communication)
1. Student stays focused and listens closely during class discussions. Student works well with others, sharing ideas and observations clearly.
2. Student listens closely to class discussions. Student gets along with others in the group but does not participate in discussions.
3. Student has difficulty staying focused and listening during in-class discussions. Student has difficulty working with others in a group situation and/or does not participate in discussions.
Imagination or Creative Production
1. Student assists in creation and demonstration of rhyming patterns, words, and phrases. Student demonstrates a thorough understanding of rhythm and meter through recognition.
2. Student is able to understand simple rhythm patterns but has difficulty with rhyming words and phrases. Student is able to recognize some rhyming words and phrases, but is inconsistent with rhythm patterns.
3. Student has difficulty with, or is unable to distinguish rhyming words and phrases or rhythm patterns.
Behavior and Participation
1. Student is able to stay focused on the task and participates and tries to reach his/her personal best. Student encourages other students to participate and reach their personal best.
2. Student is able to stay on task most of the time and works hard to achieve his/her personal best. Student encourages other students to reach their personal best.
3. Student is able to stay on task most of the time and works hard to achieve his/her personal best. Student is not encouraging to other students and does not help them reach their personal best.
The same format is used for the Sound Exploration Unit as well.
